Medical Information for Student-Athletes and Parents

It is extremely important for student-athletes and their families to read and understand the procedures and expectations followed by the sports medicine department for the management and referral of athletic related injuries and illnesses (Injury/Illness Management & Referral form) and the processes followed for the coverage any medical expenses incurred from medical referrals (Medical Referral and Expense Coverage form).

Student Athlete Pre-Participation Paperwork

Each student-athlete, new or returning, must compete REQUIRED pre-participation paperwork prior to beginning any team conditioning, training, or competitive activities. Barton CC cannot assume any responsibility for medical referral claims until this paperwork has been completed.

Each athlete must complete an athlete profile in the Athletic Trainer System (ATS) Athlete Portal

New student-athletes: Complete your ATS Athlete Profile in its entirety prior to August 1. (Instructions to complete your ATS Athlete Profile  Video Instructions).

Returning student-athletes: Complete your ATS Athlete Profile in its entirety prior to August 1. (Instructions to complete your ATS Athlete Profile  Video Instructions). 

Physicals: Required pre-participation physicals will be completed by Barton Sports Medicine Physicians in August. Coaches will be provided the schedule as it is completed.
